Meet the Founder

Keshona Isaacksz is a board certified family nurse practitioner and the Founder and Clinical Director of Inherited Health Medical Center. Her professional background spans wound care, foot care, chronic disease management, mobile healthcare, primary care, clinical operations, and care for medically complex patients.

Prior to founding IHMC, Keshona served as a Medical Director, where she developed experience in clinical leadership, care coordination, and the operational aspects of delivering healthcare to patients with complex and ongoing medical needs.

Keshona founded Inherited Health Medical Center with a clear purpose: to improve access to individualized, compassionate healthcare for patients who face barriers to traditional office-based care. She recognized that many adults — particularly older adults, homebound patients, and those with mobility limitations — were not receiving the wound care and foot care they needed simply because they could not easily travel to a medical office.

Her clinical approach is attentive, educational, and collaborative. She takes time to listen to patients and their families, explains care plans in clear and understandable terms, and works closely with physicians, specialists, home health agencies, and care facilities to support continuity of care. She believes in treating the whole person — not just the wound or the foot — and in involving patients and caregivers as active participants in their own healthcare.
